Discontinuing Medications in a Patient Chart
There are two actions available to discontinue an active medication in a patient’s chart. The correct action depends on whether the medication was discontinued by you/your practice or by another prescriber or the patient.
Discontinue vs. Document Discontinue
- Discontinue: Use when the patient is no longer taking the medication and it is being discontinued by you or your practice, including when treatment is complete.
- Document Discontinue: Use when you are documenting that a medication was discontinued by another provider or by the patient.
Important: A cancellation request to the pharmacy should be used in conjunction with a discontinue action and is only available if the medication was originally prescribed from Elation.
To Discontinue a Medication Prescribed by You or Your Practice
- Locate the medication in the patient’s Clinical Profile, Chronological Record, or Medication List.
- Click Actions next to the medication name.
- Click Discontinue.
- Select the most appropriate discontinue reason:
- I want to discontinue this medication
- Discontinued by another prescriber
- Patient stopped taking medication
- Verify the Discontinued by field:
- If I want to discontinue this medication is selected, your name will appear if you are a Provider-level user.
- If you are an Authorized Rx Delegate, you must select the provider you are acting on behalf of. Authorized Rx Delegates can only discontinue medications for providers they are delegates for.
- Verify the Discontinued date.
- (Optional) Enter additional details in the Additional reason field.
- This field is not transmitted to the pharmacy if a cancellation request is sent.
- (Conditional) If the medication was previously sent electronically from your practice and the pharmacy accepts electronic cancellations, check Send a cancellation request to… to notify the pharmacy.
- Click the blue Discontinue button to save.
Note: The button label may vary depending on the discontinue reason selected and whether a cancellation request was sent.
To Document a Medication Discontinued by Another Prescriber or the Patient
- Locate the medication in the patient’s Clinical Profile, Chronological Record, or Medication List.
- Click Actions next to the medication name.
- Click Discontinue (may appear as Document Discontinue)
- Select the appropriate discontinue reason:
- Discontinued by another prescriber
- Patient stopped taking medication
- Verify the Discontinued by field.
- (Optional) If Discontinued by another prescriber was selected, enter the name of the prescriber.
- Verify the Discontinued date.
- (Optional) Enter additional details in the Additional reason field.
- This information is not transmitted to the pharmacy if a cancellation request is sent.
- (Conditional) If eligible, check Send a cancellation request to… to notify the pharmacy.
- Click the blue Discontinue button to save.
Note: The button label may change based on the selected discontinue reason and whether a pharmacy cancellation was included.
