Overview
Internal referrals allow PACE staff to initiate services such as Occupational Therapy, Physical Therapy, Social Work, and other internal disciplines directly from the EMR and have the referral automatically appear in CareHub for service tracking.
This article explains how to create and submit an internal referral and where to find it in CareHub after submission.
Before You Begin
- Your organization should have a PACE Internal Providers option configured during implementation.
- Internal referrals are created in the EMR and then automatically sent to CareHub.
- No authorization is required for internal referrals.
Create an Internal Referral
Option 1: Create a Referral from a Visit Note
- Open the participant record in the EMR.
- Start or open a visit note.
- Select Referral from within the encounter.
Option 2: Create a Referral Directly
- Open the participant record.
- Select Referral from the available options on top toolbar.
- Complete the referral details.
Complete the Referral Information
- Select PACE Internal Providers as the referral destination.
- This acts as a generic internal referral and is not routed to a specific individual.
- Select the appropriate discipline (for example, Occupational Therapy).
- Enter the referral details.
- Type the information manually or
- Use Wordsmith if desired or
- Apply configured templates if your organization uses them
Submit the Referral
- Review the referral information.
- Select Sign and Send.
Once submitted, the referral is automatically sent to CareHub. No printing is required.
Verify the Referral in CareHub
- Return to CareHub; either Service Tracking module or Participant Profile
- Locate the new referral.
The referral will display Provider as PACE INTERNAL (discipline)
Type PACE in the search field to filter to internal referrals
Participant Profile > Service Tracking
Service Tracking > Services
Begin Service Tracking
After the referral appears in CareHub:
- Open the referral record.
- Review the service details.
- Select Add Visit to begin managing services and associated visits.
Initial Status
New internal referrals will:
- Display a status of Not Started
- Have no visit placeholders created
- Be ready for service tracking activities
Important Notes
Internal referrals do not require authorization.
A referral submitted through the EMR is automatically available in CareHub for service tracking after processing.
Visit management and ongoing service tracking are completed within CareHub after the referral is created.
