Overview
The Huddle is the daily meeting workspace for the Interdisciplinary Team (IDT) in CareHub. Designed to support all required PACE disciplines—including Primary Care, Nursing, Social Work, Therapy, Dietary, Home Care, and others—the Huddle provides a structured agenda for reviewing participant updates, recent events, and care coordination needs.
As participants are discussed, meeting facilitators can create follow-up tasks and assign them to the appropriate team members. Examples include:
- Clinical visits
- Home visits
- Medication reconciliations
- Assessments
- Other participant care follow-up activities
This helps ensure coordinated communication and transparency across the care team within a single application.
Adding Participants to the Huddle
Participants can be added to the Huddle either automatically or manually.
Automatically Added Participants
Participants may be automatically added to the Huddle based on specific events, including:
- New enrollments
- Disenrollments
- Recent admissions (Hospital or SNF)
- Incidents (for example, falls)
Manually Add a Participant
Any user can manually add a participant who was not automatically included.
Navigation:
IDT Meeting > Huddle > Actions > Add Participant
Discussing Participants
Selecting a participant from the Huddle opens the Participant Chart, which provides information to structure the discussion.
The Participant Chart includes:
- Profile
- Timeline
- Tasks
- Medication Profile
- Conditions
- Care Plan
- Quality
- Assessments
- Authorization
Managing Discussion Status
Once the participant discussion is complete, choose one of the following options:
Mark as Discussed
Use this option when the participant has been reviewed during the Huddle.
Navigation:
Huddle > Participant > Chart > ⋮ (More Options) Next to Note > Set IDT Note Attendance
Move to Another Day
Use this option to carry the participant forward to the next day's Huddle.
Navigation:
Huddle > Participant > ⋮ (More Options) > Move to Another Day
💡 Best Practice Tip
At the conclusion of the Huddle, review any tasks that were created during participant discussions. These tasks are available under:
IDT Meeting > Review Tasks
Reviewing tasks before ending the meeting helps ensure follow-up activities are assigned and tracked appropriately.
