- This is the only address field that syncs, as Elation supports a single address field.

- Syncs as a tag on the patient profile in Elation. More information on Patient Tags: INFO: Participant (Patient) Code Status

The following fields do not sync due to system limitations:

Medicare ID
Medicaid ID
Race and Ethnicity
- Only CareHub supports all CMS-required race and ethnicity options.
