What are CCD and CCDA files?
CCD (Continuity of Care Document) and CCDA (Consolidated Clinical Data Architecture) files are standardized formats used to securely exchange patient medical records between certified electronic health record (EHR) systems, including Elation.
CCDA files may include:
- Demographics
- Allergies
- Problems
- Family and social history
- Immunizations
- Medications
- Labs, notes, imaging, and discharge summaries
The system has the ability to import from these files demographics, allergies, problems, immunizations, and medications as structured chart data. All other content is viewable in a Patient Summary report.
Why use CCD and CCDA files?
CCD and CCDA files allow providers and patients to securely share clinical information when care is transferred between different providers or EHR systems. They can be used to:
- Share clinical summaries with outside providers
- Exchange data between EHRs
- Submit data to registries or health information exchanges (HIEs)
- Support quality reporting
How do CCD or CCDA files get to Elation?
Patient Summary data can be received via an external vendor sending to the PACE organization or manual upload from a file on your computer. Only CCD or CCDA files from Certified EHR systems are supported.
The file will appear as a Patient Summary in the Requiring Action section.
