About These Assessments
The assessments listed below are the standard clinical assessments embedded within CareHub. These assessments can be applied on an individual (singular) basis for a participant or included as part of a larger assessment, such as the Comprehensive Assessment or Semi‑Annual Assessment.
Each assessment may be completed on its own or within a broader assessment workflow, depending on clinical needs and timing. While flexible in how they are used, our recommendation is that these assessments are always completed within CareHub to ensure consistency, accurate documentation, and alignment with reporting and care coordination workflows.
Abbreviated Mental Test Score (AMTS)
A short cognitive screening tool used primarily in older adults to quickly assess orientation, memory, and attention. It is commonly used to screen for cognitive impairment, delirium, or dementia in acute and geriatric settings.
