HOW TO: Finalize and Send Care Plan for Signature

Once the IDT team has finalized the draft Care Plan, it can be sent for signature. The sign-off process is fully integrated into CareHub to ensure a seamless experience. Each team member assigned to sign the Care Plan will receive a task in their Tasks Inbox and can easily complete the sign-off directly from their task list.

Important notes

✔ The Care Plan is not considered complete until all required signatures are received. Receiving timely signatures is essential to remain in compliance.

✔ Once the care plan is moved to signature, the IDT is able to move it back to an editable phase. Note: this action will remove all previously collected signatures, and the sign-off process must be restarted.
