Overview
Patient tags in CareHub and Elation provide a quick, at-a-glance indicator of a Participant’s advanced directives status. However, the tags themselves do not store the underlying documentation. This article explains where to upload and reference the actual advanced directive and consent documents in both CareHub and Elation.
CareHub: Uploading Advanced Directives
In CareHub, advanced directive documents should be stored directly on the Participant’s profile.
How it works:
Upload the advanced directive file to the Participant Profile
Once uploaded, the document will be available under File Attachments
This ensures the document is easily accessible from the Participant’s record while allowing tags to serve as a visual reference.
Elation: Uploading Advanced Directives and Legal Documents
In Elation, advanced directives and consent forms are stored in the Legal section of the patient chart.
Legal Section Overview
The Legal section allows users to:
Document legal notes for the patient
Upload and reference legal documents within the chart
Uploading Legal Documents
You can add legal documents using the drag-and-drop feature. Documents uploaded to the Legal section will appear under the Legal Reports category within the patient’s Clinical Profile.
This section is commonly used for:
Advanced Directives
Consent forms
Release forms
Intake forms
Patient identification (e.g., driver’s license, passport)
Other legal documentation
Document Tags
When uploading documents in the Legal section, you can apply Document Tags. These tags make it easy to identify:
Which legal documents are on file
Which documents may still be missing for a patient
Why Advanced Directives Appear in the Legal Section
Advanced directives and consent forms are typically configured and mapped as legal-type documents in Elation. Because of this configuration:
Any documents filed under Directives or Consent report categories automatically appear in the Legal section
This ensures consistent storage and visibility across patient charts
