INFO: Enrollments

The Enrollments Module is designed to support the end-to-end management of participant enrollment, from initial referral through intake and confirming an enrollment date. This module enables teams to efficiently track enrollment status, manage required documentation, and ensure compliance with program timelines and regulatory requirements.

NOTES: 

• if your implementation includes data migration from another EMR, your current active PACE Participants will be enrolled during the Implementation process and be active when you get access to your CareHub account.
• only PACE Participant should be enrolled in CareHub.  Any patients/individuals that are Adult Day Center only should be added directly to the clinical record (Elation EMR).
• Once Enrollment is completed/confirmed, the clock begins on the due date of the initial comprehensive assessment.   Organizations should determine their own internal process for completing the enrollment accordingly to the enrollment date.  For example, a referral is processed in June, but the PACE Participant isn't enrolled until July 1st.  The enrollment should be completed on July 1st.
