Any user is able to add a service determination request (SDR) into CareHub when an SDR is received from a participant, participant's designated representative or participant's caregiver. When adding a SDR, users must include the following information:
- Participant Name
- Assigned To
- Date of Service Request
- Request Category
- Service Type
- Request Description
- Submitted By
- Relationship (if not participant)
- Phone Number for Submitter
Important Notes
- Make sure the date of service request is accurate as additional date will be automated based on that date.
- All SDRs should be added into CareHub in a timely manner to ensure compliance with request timelines.
- Once an SDR is added, the participant will automatically be added to the next day’s morning meeting huddle queue under the SDR category.
