Organization	Text	The high level organization name.
Contract Number	Text	The H number of the organization.
Center	Text	The name of the specific site to which the participant is attributed.
Team	Text	If applicable, the team to which the participant is attributed.
PCP Name	Text	The name of the primary care provider to which the participant is attributed.
Elation Patient Id	Text	The ID of the patient in Elation.
Member Id	Text	The PACE organization’s member id for the participant.

DOB	Date	In the format of MM/DD/YYYY.
Encounter Date	Date	In the format of MM/DD/YYYY.
Note Name	Text	Name of the note as shown in Elation.

Note Status	Text	Options signed unsigned amended
Created By	Text	Name of person who created the note.
Signed By	Text	Name of person who signed the note.
Signed Time	Datetime	Timestamp of when the note was signed
Update At	Datetime	Timestamp of when the note was last updated.
Time With Patient	Text	Time with patient as recorded in Elation.
Time Documenting	Text	Time documenting as recorded in Elation.
