OVERVIEW: CareHub - Elation Data Flow

This document outlines how data syncs between CareHub and the Clinical EMR (Elation) and where specific patient information and clinical data should be documented.

CareHub - Elation Sync Direction Legend

Participant & Provider Data

Data Type	Direction	Notes
Participants (Creation, Enrollment, Disenrollment)	CareHub → Elation	Participants should be created in CareHub to ensure proper synchronization into Elation. Enrollment and disenrollment updates automatically flow into Elation.
Demographics, Participant Address, Emergency Contact	CareHub → Elation	Detailed Demographics when a participant is created or updated in CareHub will sync to Elation.  Note: See INFO: Participant Demographics Updates - CareHub vs Elation Sync for specific Demographic fields that DO NOT flow into Elation from CareHub
Primary Physician/Assigned Provider	CareHub ↔ Elation	The Primary Physician in CareHub syncs with the “Provider assigned in Practice” field in Elation. Updates made in either system will sync to the other. Best practice is to update the Primary Physician in CareHub.

Social History Data

Literacy Level, English Fluency	CareHub → Elation	All Social History fields should be updated in CareHub only. Literacy Level and English Fluency are the only two fields that flow into Elation from CareHub
Highest Level of Education Completed, Marital Status, Religious Preference, Citizenship Status, Veteran Status	No Integration	These fields do not flow into Elation from CareHub. All Social History fields should be completed in the Participant Profile in CareHub

Tags & Classifications

Clinical Data

Data Type	Direction	Notes
Vitals	Elation → CareHub	Vitals documented within the visit note in Elation populate in CareHub and display in the vitals trend graph. Note: The Visit Note must be signed before it will populate in CareHub
Medications	Elation → CareHub	Active medications documented in either the Visit Note or face sheet in Elation will populate to the face sheet and then flow into the CareHub Timeline and Medication tabs. Active medications are those listed as Permanent or OTC in Elation (not Temporary).
Immunizations	Elation → CareHub	Vaccinations added into Elation will populate into the Immunizations Report in CareHub.
Allergies	Elation → CareHub	Allergies are pulled into the CareHub Medications tab from the Allergy list in the Elation face sheet only. Note: Allergies documented in Visit Notes only do not sync.
Lab Results	Elation → CareHub	Lab results populate in Elation and flow into CareHub, including lab name, date, result, reference range placement, and actions. Multiple results for the same test generate a trend graph in CareHub.

Documentation & Templates

Visit Note Templates	Elation → CareHub	Templates built in Elation sync to CareHub and can be attached to scheduled appointments. Users can initiate visit notes from CareHub based on scheduled appointments. Note: Visit Notes that are tagged with "Incident" will appear in Quality Inbox > Incidents the following day after an overnight refresh.

Assessments

Assessments	No Integration	Standard Assessments should be completed in CareHub. OVERVIEW: Common Clinical Assessments in CareHub
PCP Assessments	Elation → CareHub	PCP Assessments are the only type of Assessment that should be completed in Elation. Once the PCP Assessment is completed in Elation, it can be uploaded to CareHub.

Orders & Referrals

Orders (Labs, Imaging, Cardiac, Pulmonary, Sleep)	Elation → CareHub	Creating an order in Elation generates a corresponding service tracking item in CareHub. Note: Creating a service tracking item in CareHub does not create an order in Elation.
Referrals	Elation → CareHub	Creating a referral in Elation generates a corresponding service tracking item in CareHub. Note: Creating a service tracking item in CareHub does not create a referral in Elation.
